Anticoagulant Clinic Protocol for the Referral of GP Patients
For all patients: AF Protocol or new to the area
Fully complete the Anticoagulant referral form, which can be printed off from the link below. An NHS number is mandatory
For AF Protocol
- Baseline bloods (FBC,U&E, LFT, INR, APTR) and Blood Group (unless already documented) to be taken and reviewed by referring doctor prior to commencing anticoagulation. Significant abnormalities should be discussed with a Duty haematologist prior to commencing anticoagulation.
- Give the patient a prescription of the three different strengths of warfarin (1mg, 3mg & 5mg). The patient must then be loaded on 3mg each evening for six days prior to their counselling appointment.
- You will need to phone the Anticoagulant clinic on 0118 3227691 or 0118 3228237 to book an appointment with the clinic nurse for counselling when appropriate.
- Once the appointment is booked with the clinic the Anticoagulant referral form must then be faxed to the department on 0118 3228374.
- The patient will have a blood test taken at their appointment in the clinic. They will also be given an ID card, information pack and ongoing dosage instructions.
- The patient should arrange subsequent blood tests at their preferred location.
