Procedure for Assessment of Out-Patients on Anticoagulant Therapy with Bleeding and/or Loss of Anticoagulant control
- This document serves as a procedure to be followed by the Anticoagulant Clinic nurses/medics in the event of a patient being identified as high risk of severe haemorrhage whilst on oral anticoagulation therapy and/or as a result of loss of anticoagulant control as an out patient.
- If a patient has an INR >8.0 and/or major bleeding they may require assessment at the Clinical Decision Unit (CDU) – Inform the patient and assess if requires a chair or trolley and check they have transport to get to the hospital.
- Contact the bed manager on CDU ext 8911 with the name/ DOB/Hospital No and telephone number of the patient giving relevant clinical details.
- Anticoagulant Clinic staff to contact patient again and advise them to attend CDU. Contact GP to arrange transport if required.
- Anticoagulant Clinic will fax across a referral letter (from the DAWN anticoagulant system) to the CDU on Ext 6938 prior to patient’s arrival for the Medical registrar.
- Once admitted the medical team will assess and treat the patient as per protocol Recommendations for the Management of Bleeding/Excessive Anticoagulation. See link

- Note: If no bed available on CDU the patient should be asked to attend A&E
