Anticoagulation Clinic
A copy of the procedure for referral of patients on oral anticoagulants to the anticoagulant clinic is available on all wards.
For patients started on oral anticoagulant therapy during a current admission.
- 1.) Telephone the Anticoagulant Clinic on ext 7691 to book the first appointment. Clinics are run Monday to Friday with the exception of Bank Holidays. We do not insist on seeing all patients if, for example, they are immobile or live far from the RBH. The first blood test should be arranged through the patient's surgery or District Nurse.
- 2.) Complete a yellow anticoagulant referral form and send it to the laboratory. If we do not receive this form we will not be able to take on the patient.
- 3.) A patient's anticoagulant treatment is the responsibility of the discharging hospital clinical team until the agreed date of the follow up appointment by the Anticoagulant Clinic. A discharge dose must therefore be prescribed and an adequate supply of anticoagulant be dispensed before the patient leaves hospital (advice is available if required).
- 4.) The patient should also be given a yellow Anticoagulant discharge card and an information leaflet about the clinic, supplies of which are kept on wards.
- 5.) If the patient is to be discharged to another hospital e.g. Townlands, Wokingham, Sue Ryder or to a Nursing Home please complete a yellow referral form and inform the coagulation section (ext 7691).
For patients on oral anticoagulant therapy prior to admission.
- 1.) These patients do not need to be seen at the clinic so please send a yellow referral form as soon as possible.
- 2.) The patient can then be given a discharge dose and a date for a next test.
